Eligibility
- Employed individuals from 18 years to below 60 years
- Employed individuals with a net salary of LKR 500,000 and above (Elite Circle status) or
- Employed individuals with a net salary less than LKR 500,000 and above LKR 35,000/-.
Documents Needed
- Duly completed application form.
- Duly completed declaration form for the insurance as applicable
- Undertaking to remit salary to Union Bank
- Proof of identity (NIC / Passport), Salary (salary slip within last 3 months) & proof of address (utility bill / bank statement within last 3 months)
- Completed VISA Debit Card application
